Any requests containing personal information should be made by telephone, by fax, through traditional mail service, or in person (by appointment).

Level 1

If you have feedback, questions, comments or issues, our management team will be pleased to speak with you about them. Our managers are authorized to resolve your issue in a timely and effective manner.

Level 2

If your issue is complex or not resolved to your satisfaction, the manager will involve the area director. The director has the authority to evaluate the analysis of the issue to determine the cause and ensure APS will deliver on its commitment to excellent service.

The director will also advise the Client Service Advocate of the issue and its progress.

Level 3

If further investigation of the issue is required, the investigation will be overseen by the Client Service Advocate. The Advocate provides assurance to you that APS will deliver on its commitment to excellent client service.
